Breast Cancer Survivorship Clinic
The Survivorship Clinic allows you to meet with breast cancer survivorship team members and create a plan to promote your health and lessen late effects of cancer and treatment. During your visit you will receive:
- A written summary of your cancer treatment;
- A survivorship plan that includes recommendations to promote health and lessen the impact of late effects of cancer and treatment;
- An assessment of your individual physical, practical and psychosocial needs by a nurse practitioner and a social worker;
- Information and skills you need to manage post-treatment side effects such as fatigue and lymphedema, or swelling;
- Access to a dietitian to discuss nutrition and exercise, and address your personal concerns about returning to a healthy lifestyle after treatment for breast cancer;
- Community resources to address financial, employment and insurance issues.
The Survivorship Clinic is held at the Chagrin Highlands Health Center in Orange, Ohio. Spouses or partners are encouraged to participate.
