IRB General Information
IRB Scope and Responsibilities
The UH IRB reviews all human subject research protocols that originate from:
1) University Hospitals and Center for Clinical Research staff members,
2) the departments housed in the CWRU School of Medicine,
3) the School of Nursing, Dentistry or any other department that involves patients or personnel of the hospital, as well as,
4) all projects involving the hospital's patients or personnel, regardless of sponsor, and
5) selected proposals as requested by the CWRU IRB.

Meeting Dates and Submission Deadlines
The IRB meets every Tuesday (except holidays) at 3:30. There are two separate boards that meet every 2 weeks. The deadline for protocol submission is 10 days before the meeting date.

Exemption Request submission form update - Friday, June 20, 2008

A new Exemption Request Form (version 06/08) has been created for use. This new form combines the 2 previously required forms: “Exemption Determination Form” and “Request for Exemption” . Please note that all exemptions must receive Department Research Review Committee/ Department Chairperson review prior to submission to the IRB. A signature block for this approval has been added to page 6 of the new form.

Please note that per UHCMC IRB policy, projects which are approved as “Exempt” will be awarded an expiration date of six (6) years from the time the exemption was approved. If the research is ongoing for six years the investigator must resubmit the research for review as a new protocol

 

HIPAA Authorization Template Update - Monday, March 03, 2008
The UH Privacy Office recently moved to new office space in the Management Service Center in Shaker Heights.  As a result, the UH HIPAA Authorization template has been revised to reflect the Privacy Office's new address.  Please use the new version of the HIPAA Authorization template (v021808) when submitting new protocols.  Beginning April 1, 2008, please begin submitting revised HIPAA Authorization forms for all actively enrolling studies at the next Amendment or Continuing Review.
