Skilled Care Facility/Short-Term Rehabilitation

Our Skilled Care Facility  provides sub-acute care. Sub-acute care is goal-oriented, comprehensive, inpatient care for the medically complex patient who has had an acute illness, injury, or the exacerbation of a chronic disease. This care is administered immediately after or instead of acute hospitalization. Sub-acute care settings allow the patient to receive complex care or treatment with a focus on rehabilitation to return to prior functions. Using an interdisciplinary approach, the sub-acute program provides a person-centered care encouraging each patient to participate in their care plan.

The sub-acute staff is made up of physicians, nurses, and therapists and qualified support personnel such as nursing assistants, therapy aides, and technicians, all highly skilled professionals. Because of the range of staffing expertise, sub-acute care is more intensive than traditional nursing home care, but less intensive than acute hospital care. The length of stay can range from 20 to 100 days, depending on the complexity of the patient's condition and the length of time for stabilization or completion of the pre-determined treatment plan. We help each patient to return home as soon as safely possible.

The sub-acute is accredited by the Joint Commission on Accreditation of Healthcare Organizations (JACHO), a certification that few skilled nursing centers acquire. Our sub-acute center has received zero deficiencies on the state survey for care.

Admission can be coordinated by a hospital representative upon hospital discharge or prescribed by your physician within 30 days of a hospital stay.

Funding sources include Medicare, private insurance, and Medicare HMOs.
